All-School Health Policy


To ensure the well-being of all our students, the following is required by the State of Delaware or by The Independence School:
 

All NEW students to the school (ECi, ECii, K and grades 1-8):
  • Physical exam with tuberculosis screening or mantoux test within the past 12 months
  • Immunization record current with state requirements
  • Varicella Vaccine AND Booster is required for all NEW students entering K-8th grade
  • Exemptions include physician verification of disease, medical or religious
  • Proof of lead screening required for ECl, ECll and K ONLY
All new and current students (K and grades 1-8):
  • Immunization record current with state requirements
  • Completed Hepatitis B series
  • Varicella Vaccine and Booster
  • Exemptions include physician verification of disease, medical or religious
ECi, ECii and K students:
  • Yearly physical exam with tuberculosis screening
  • Immunization record current with state requirements
  • MMR between 4-6 years
  • Proof of lead screening (mandatory)
1st-grade students:
  • Current physical exam with tuberculosis screening
  • MMR booster may be given at this time
3rd, 5th & 7th-grade students:
  • Physical exam required with tuberculosis screening
  • MMR booster may be given between the ages of 10-12 if not given at ages 4-6
7th & 8th-grade students playing sports:
  • Students will NOT be allowed to participate in sports without a Delaware Interscholastic Athletic Association (DIAA) DIAA Physical Evaluation form on file.
  • The DIAA Pre-Participation Physical Evaluation and Consent Form consists of seven (7) pages. The student must be cleared to participate on or after April 1 based on a physical examination conducted within 12 months of the physician’s signature. The clearance is valid through June 30 of the following year. Any students who will participate in sports must hand in their physicals BEFORE the first day of TRYOUTS.
